What Changes in These Photographs
The variable is firmness, not dimensions. A penile prosthesis replaces the function of the erectile chambers, so the after image shows an erection the patient produces himself, whenever he chooses, for as long as he wants. That control is the part a photograph cannot convey, and it is the reason the International Society for Sexual Medicine reports satisfaction above 95 percent in this operation.
The procedure is not experimental. It has been performed for roughly 50 years and more than 30,000 devices are implanted around the world each year. What separates one result from another is rarely the device: it is the condition of the tissue at the time of surgery and the accuracy of the sizing.
When Each After Photograph Was Taken
Sexual activity is permitted at six weeks with an inflatable prosthesis and at four weeks with a malleable one. The after images here were taken once that period had passed, because a photograph recorded earlier shows swelling rather than an outcome.
A minority of men perceive a shortening of 1 to 2 cm. The cause sits before the operation, not in it: years of erectile dysfunction cause the corpus cavernosum to atrophy, and the implant is sized to the tissue that remains. Modern technique does not shorten the penis on its own. Knowing this before surgery is what separates a satisfied patient from a disappointed one.
