How the Angle Decides the Operation

Curvature is graded in three bands: mild under 30 degrees, moderate between 30 and 60 degrees, and severe above 60 degrees. A mild bend usually needs nothing. A moderate one is operated when intercourse has become difficult. A severe one is a surgical candidate in almost every case.

Direction is the second half of the assessment. Around 50 percent of curves bend downwards, 30 percent sideways and 20 percent upwards, and that decides where the correction is placed. Both figures come from the examination rather than from the patient's own estimate, which is why an erect photograph is requested before the consultation. What Correction Costs, and What It Does Not

Plication and Nesbit techniques straighten the shaft by shortening the longer convex side, so a perceived shortening of 0.5 to 1.5 cm is expected. It is perceptual rather than truly anatomical, and the large majority of patients accept it in exchange for a straight erection.

Sensation is the concern men raise most often, and the figures are reassuring in the group most likely to be operated: 2 to 5 percent in young and otherwise healthy patients, usually recovering within 3 to 6 months where it does occur. Careful preservation of the neurovascular bundle is what keeps that number low. Six weeks without intercourse afterwards is not caution for its own sake: the tunica albuginea needs that time, and an early return is a common route to a recurrent curve.
